- Must come in with a face covering
- Please come at the exact time of your appointment
- New Patients must register prior to appointment (fill out paperwork here)
- Come alone unless you have a child or at home attendant / aid
- If you have COVID19 symptoms, you must disclose this to us prior to your visit; ie: fever, cough, loss of taste
